Information that will be needed when completing the Estate Planning questionnaire powered by Legal Karma, provided through MMFCU’s Online Banking or Mobile App for members includes:
- Children’s information (Full Legal Name and Date of Birth)
- Spouse’s information (Full Legal Name and Date of Birth)
- Personal information for any person or entity that you would like to make an agent for any of the listed positions (Full Legal Name and Date of Birth)
- The Name, Address and Phone Number for any charitable or religious entities that you plan to list as a Beneficiary
- Health Care Provider Information (Name, Address and Phone Number)
- Preferred Mortuary/Internment provider, if known (Name, Address and Phone Number)

- How long does it take to complete the estate planning questionnaire? On average, it takes 30 minutes to complete the Will Package and 45 minutes to complete the Trust Package.
- Who do I contact if I have questions about the estate planning documents or process?

- Can the questionnaire be completed on a mobile device?
Yes, both MMFCU's Online Banking and Mobile App have the ability to complete the Wills and Trusts forms.
- Can I amend my existing estate documents? Yes, if your existing documents were created through Mid Minnesota Federal Credit Union using Legal Karma, we can support amendments. You can make changes twice in the first 6 months for free.
- After downloading the documents and signing them with the notary, etc., where should I store the documents? These important legal documents are best stored in a safe and accessible location. You may want to consider a safe deposit box. Appointed agents may need to present the documents showing their authority in order to act on your behalf, or on behalf of your estate. Copies of your medical power of attorney and living will/advance directive can be provided to your agent, in case of emergency. If you have created a revocable trust, extra copies of the trust certificate will be helpful when interacting with your financial institutions, title companies, etc. for trust funding purposes.
- If I move to another state, do I need new documents? Every state has its own laws and standards for estate planning documents. It’s best to confer with an estate planning attorney admitted to practice in your new home state.
